My weapon of choice for this blog is Serendipity Weblog System.
The criteria were simple:
Serendipity seems to have all 6 of the above. It is a pain to install the way I did it. I'm mod_rewriting the /serendipity/ directory away and it causes a ton of pain. I managed to do that, but it is obvious that the authors did not plan this software to be (ab)used like that.

That's strange - it should be no problem to install s9y at the top of your docroot. Just unzip/untar/copy all files to the right place (or even set your docroot to the serendipity directory.
If you change an existing installation, you may have to adjust your "Full path", "Relative path" and "URL to blog" (at "Configuration" -> "Path" in the backend) or "serendipityPath", "serendipityHTTPPath" and "defaultBaseURL" in the serendipity_config database table to the new values.
